Clinical Document Management

Structured handling of incoming clinical correspondence, from review through to filing on the patient record.

Overview

Practices receive a constant flow of letters, results and other clinical documents. CDS reads that correspondence, classifies it, completes coding or actions that the protocol allows, and files it so it can be retrieved later.

The aim is an organised record: documents attached to the correct patient, actions identified, and information coded where a code is required.

Workflow

  1. 1Document Received
  2. 2Clinical Review
  3. 3Classification
  4. 4Coding / Action
  5. 5Record Filing

Incoming correspondence

Letters and clinical documents are reviewed rather than left in a backlog.

Appropriate coding

Information that needs a code is translated into SNOMED CT or Read codes.

Required actions

Actions follow the shared protocol. Anything outside that protocol is returned to the practice.

Organised records

Filing is structured so documents can be found and records are ready for review or audit.
